Sample Admission Notes for the Most Common Conditions

Simplify you life. For example, let's imagine that you have a patient complaining of leg pain in front of you. There only so many common causes of leg pain but it is easy to miss one or two when you are busy multitasking. Print these sample notes and there you have all the questions to ask, the common physical findings, even the assessment and plan. Just tailor the admission note to your patient. Having the basics pre-written in front of you will help you to pick-up the unusual and challenging findings. Do not bother to concentrate on starting Heparin and GI prophylaxis, they are already there, in the note, as a reminder. Save your clinical acumen for the real challenges.
